چکیده
IntroductionMycotic aortic pseudoaneurysms (MAP) are a rare but often fatal complication of thoracic surgery. Recently, our institution has transitioned to performing all lung transplants on venous-arterial extracorporeal membrane oxygenation (VA-ECMO) support when possible. Reinforcement cannulation sites with pledgeted sutures is necessary control bleeding; however, pledgets could also represent an iatrogenic risk for infection. Here we present case MAP at the site in transplant recipient (LTR).Case ReportWe 56-year-old female who underwent bilateral scleroderma-related interstitial disease. Two months following transplant, she was admitted methicillin-resistant staphylococcus aureus bacteremia. Initial echocardiogram inconclusive endocarditis and thus treated IV vancomycin six weeks clinical improvement. Unfortunately, two later presented recurrent imaging revealed evidence ascending formation, confirmed by positron emission tomography (Figure 1). She surgical repair identification palpable pathology consistent acute organizing inflammation microabscess formation. Despite aggressive medical management, patient suffered catastrophic embolic stroke.SummaryWhile ECMO many potential benefits, it not without risk. Closure immunosuppressed patients may lead development infected can have devastating consequences. To knowledge this first pledget LTR. Mycotic (LTR). We stroke. While

Mycotic pseudoaneurysm associated with aortic coarctation.
Development of a mycotic aneurysm or pseudoaneurysm after subacute bacterial endarteritis is uncommon. Nonetheless, patients with coarctation of the aorta are more likely to develop this complication. We describe a case of a large pseudoaneurysm discovered in a child with a previously undiagnosed aorta coarctation. Successful repair was performed with the aid of partial left heart bypass and th...

Mycotic aneurysm is an established condition first identified in 1885 by Sir William Osler. It is linked to malignant endocarditis. With prevalence of 0.7-2.6% of all cases of aortic aneurysms, it is associated with a significant rate of mortality and morbidity. Physicians should be highly cautious, as diagnosis and effective treatment for this condition are difficult. The following is a case r...

Two cases of delayed non-mycotic false aneurysm arising from ascending aortic cannulation site, presenting one-and-a-hald years and seven years after cardiopulmonary bypass, are described. These two cases represent an incidence of 0.12% of this complication. Repair using profound hypothermia and circulatory arrest with femoral artery and femoral vein cannulation for cardiopulmonary bypass is re...

INTRODUCTION This observational report depicts typical problems of extracorporeal membrane oxygenation cannulation from a large case series of a single center. METHODS We analysed our experience with 720 consecutive patients receiving veno-venous or veno-arterial extracorporeal membrane oxygenation focusing on the spectrum of complications occurring in a subset of 159 patients treated with pe...
